Tracking Adoption Cases

Introduction

 

This article is meant to provide workers with an overview of tracking adoption cases in extendedReach. If you have a question not addressed here, you can contact support at support@extendedreach.com.

 

 

 

Tracking an Adoption Case

 

When adding a new case you can specific if you would like to track as an Adoption Case and/or if it is a Straight Adoption.

 

 

 

In the General Tab, check the box next to “Track Adoption Case”.

 

extendedReach will automatically open another check-box option called “Straight Adoption”. Check this box if this case is not placed in foster care at your agency, and only adoption services will be offered. For straight adoption cases, no foster care worker at your agency will be assigned and no traditional foster care placement will be done. You will be required to specify what agency has or is providing foster care services for the child.

 

Straight adoption cases are not included in Days Care Calculations. A Matched Family and Current Location will be identified, but there will not be a “Placement” as in Foster Care.

Adoption Case Information

 

This section will appear for foster-adopt and straight adoption cases.

Field Name Description
Adoption Admission Select a date with the calendar icon on the right side of the information box. If this is a foster-adopt case, this will be a different date from the foster care admission date.
Adoption Worker Select an adoption worker from the drop-down menu.
Adoptive Name Indicate an adoptive name, if this applies.
Family Matched Select a date with the calendar icon on the right side of the information box. This is the date when the family was selected.
Identified Family Select a home from the magnifying glass icon on the right side of the information box. For a home to be matched to an adoption case, the home must have the “Adoptive Home” checkbox checked under their provider type.
Competing Parties Check this box if there are two parties competing for this adoption.

 

Location

 

Track where the adoption will be located or currently is located. Click on the hyperlink "Location" button to pick a adoptive home in extendedReach. The home information will automatically populate the rest of the fields. 

This setting can be enabled by an Agency Administrator under Organization Preferences.

Field Name Description
Location Click on the hyperlink to access available locations which this adoption is or will be located.
Contact Name Document the name of the contact person related to the adoptive location.
Phone Document the phone number of the adoptive location.
Address Document the address of the adoptive location.

 

 

Adoption Tab

 

This tab becomes available when you Track Adoption Cases. If you add notes to this section, an asterisk will appear on the tab.

Intake and Discharge Rosters and Summaries

 

Case Rosters

 

View all open and active adoption cases by going to Case Rosters > Adoption Cases.

Intake Summaries

 

View an overview of all adoption cases that have been admitted to your agency by going to Intake > Adoption Cases.

Discharge Summaries

 

View an overview of all adoption cases that have been discharged from your agency by going to Discharge > Adoption Cases. To sort the discharged by outcome, go to Discharge > Adoption Outcomes.
