Electronic signatures are not added when approved

Approval signatures will fail to populate a document due to the below three reasons:

1. The approver's signature is not stored in their extendedReach Staff Profile.  

2. The document template does not have field codes for the signature.  

3. The supervisor field codes have been deleted by a worker, prior to the documents approval.
